Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading SNF in Tuscumbia, AL. Key Responsibilities:
• Work rotation schedule of 3 on 2 off 2 on 3 off
• Provide licensed practical nursing services in skilled nursing facility
• Work 12-hour shifts from 06:00 to 18:00
• Deliver direct patient care according to established nursing standards
• Maintain current LPN licensure and certifications
• Document patient care activities and assessments
• Administer medications as prescribed and per facility protocols
• Monitor patient vital signs and report changes to supervising RN or physician
• Assist with activities of daily living and patient mobility
• Follow infection control and safety protocols
• Collaborate with interdisciplinary healthcare team
• Maintain patient confidentiality and HIPAA compliance
• Participate in care planning and patient assessments
• Respond to patient calls and address immediate needs
• Complete required documentation in a timely and accurate manner
Job Requirements:
- Active, unencumbered license in AL
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
