Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Cottonwood, AZ. Key Responsibilities:
• Provide outpatient physical therapy services with focus on female pelvic health and preferably male pelvic health • Support patients with referrals primarily from urology and OBGYN providers • Provide care for other orthopedic patients as needed in caseload • Maintain daily patient load of approximately 11 patients • Work Monday through Friday 8am-4:30pm schedule • Maintain current Arizona physical therapy license • Maintain current BLS (American Heart Association) certification • Float to surrounding outpatient clinics as requested • Utilize Cerner charting system for documentation • Work 2 out of 3 major holidays (New Year's Day, Thanksgiving Day, Christmas Day) • Complete required compliance testing within specified timeframes • Provide two professional references with one from most recent employer • Complete required modules during first week of orientation (average 6 hours) • Adhere to business casual dress code • Comply with facility policies and procedures for patient care delivery • Participate in interdisciplinary care with interpretation services and unit secretary support • Provide care for patients requiring orthopedics, geriatrics, cardiopulmonary, neurological, lymphedema, and pelvic health services
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
