Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Cottonwood, AZ. Key Responsibilities:
• Provide physical therapy services with emphasis on pelvic health for female (required) and male (preferred) patients
• Support patients with referrals primarily from urology and OBGYN providers
• Treat other orthopedic patients as part of caseload
• Maintain daily patient load of approximately 11 patients
• Work Monday through Friday, 8am-4:30pm schedule
• Maintain current Arizona physical therapy license
• Maintain current BLS (American Heart Association) certification
• Float to surrounding outpatient clinics as needed
• Utilize Cerner charting system for documentation
• Work 2 out of 3 major holidays (New Year's Day, Thanksgiving Day, Christmas Day)
• Provide care for patients with orthopedic, geriatric, cardiopulmonary, neurological, lymphedema, and pelvic health conditions
• Complete required compliance modules and training programs during first week of orientation
• Provide two professional references covering recent employment history
• Adhere to business casual dress code requirements
• Complete laboratory testing within required timeframes
• Maintain flexibility to work at various clinic locations based on census needs
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
