Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Cottonwood, AZ. Key Responsibilities:
• Provide outpatient physical therapy services with focus on pelvic health for female (required) and male (preferred) patients
• Support patients with pelvic health diagnoses referred primarily from urology and OBGYN providers
• Cover other orthopedic patients in case load as needed
• Maintain daily patient load of approximately 11 patients
• Work Monday through Friday 8am-4:30pm schedule
• Maintain current Arizona state physical therapy license
• Maintain current BLS (American Heart Association) certification
• Utilize Cerner charting system for patient documentation
• Float to surrounding outpatient clinics as requested
• Work 2 out of 3 major holidays (New Year's Day, Memorial Day Weekend, 4th of July Weekend, Thanksgiving Day, Christmas Day)
• Complete required compliance testing within specified timeframes
• Complete billable training modules during orientation week (average 6 hours)
• Provide two professional references with one from most recent employer within 1 year
• Adhere to business casual dress code requirements
• Support interdisciplinary care with interpretation services and unit secretary
• Provide services for orthopedics, geriatrics, cardiopulmonary, neurological, lymphedema, and pelvic health conditions
• Maintain flexibility for floating between units based on skill sets and census needs
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
