Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Nights at a leading SNF in Rutland, VT. Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in a Skilled Nursing Facility setting
• Work night shift from 22:00 to 06:30
• Work both weekends as required
• Provide comprehensive patient care and monitoring throughout assigned shift
• Administer medications and treatments as prescribed
• Document patient care activities and observations
• Collaborate with healthcare team members to ensure quality patient outcomes
• Follow facility policies and procedures for patient safety and care delivery
• Maintain professional nursing standards and ethical practices
• Respond to patient needs and emergency situations as they arise
• Assist with patient activities of daily living as needed
• Maintain accurate and timely nursing documentation
Job Requirements:
- Active, unencumbered license in VT
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
