Position Summary:

Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in a Skilled Nursing Facility • Work both weekends when needed • Maintain current LPN licensure • Work 2:00 PM to 10:30 PM shift • Provide direct patient care and nursing services • Follow facility policies and procedures for patient care delivery • Document patient care activities and maintain accurate records • Collaborate with healthcare team members to ensure quality patient outcomes • Adhere to all regulatory and compliance requirements • Maintain professional standards and continuing education requirements
Job Requirements:
- Active, unencumbered license in VT
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
