Position Summary:
VitalHire is seeking a skilled and compassionate SLP - SLP to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Cottonwood, AZ. Key Responsibilities:
• Provide speech language pathology services for 85% pediatric outpatient and 15% adult outpatient cases • Conduct one day of adult acute care services per week • Maintain daily patient load of 12-14 patients • Work Tuesday through Saturday schedule (8:00-4:30) with outpatient services Tuesday-Friday and acute care on Saturday • Maintain current BLS certification throughout assignment • Maintain current CCC-SLP certification • Utilize Cerner charting system for patient documentation • Complete required compliance modules averaging 6 hours during first week of orientation • Work 2 out of 3 major holidays (New Year's Day, Memorial Day Weekend, 4th of July Weekend, Thanksgiving Day, Christmas Day) • Float between units within hospital or to affiliate hospitals based on census and skill sets • Provide two professional references with one from most recent employer within 1 year of start date • Complete required state background checks and fingerprint clearance card for Arizona • Comply with business casual dress code requirements • Complete lab testing within 72 hours of contract signing • Submit all compliance documentation by Wednesday prior to start date • Conduct speech and language evaluations and assessments • Provide treatment for various conditions including dysphagia, dysarthria, aphasia, and voice disorders • Work with patients across age groups including adolescents, adults, pediatrics, and geriatrics • Perform oral motor and oral peripheral examinations • Conduct hearing screenings as needed • Develop and implement treatment plans and discharge planning
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
