Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. Key Responsibilities:
• Provide acute care occupational therapy services to adults and geriatric patients
• Maintain current Arizona state license and BLS certification
• Complete required compliance documentation and certifications
• Work 40 hours per week following 5x8 day shift schedule
• Work required weekend coverage and 2 out of 3 major holidays
• Utilize Cerner charting system for documentation
• Conduct patient evaluations and reassessments
• Develop and implement individualized plans of care
• Provide patient and family education services
• Perform discharge planning and pre-discharge assessments
• Work with patients requiring adaptive equipment and assistive devices
• Treat various conditions including CVA rehabilitation, traumatic brain injury, spinal cord injury, and orthopedic conditions
• Manage moderate to maximum assist patients and co-treatments
• Work with ventilated patients when required
• Complete fall risk assessments and safety evaluations
• Float to other units within facility or affiliate hospitals based on census and skill set
• Complete required orientation modules and training programs
• Provide two professional references covering employment history
• Adhere to facility dress code and professional standards
• Complete required background checks and fingerprint clearance
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
