Position Summary:

Key Responsibilities:
• Provide social work services as a Licensed Master Social Worker with active WA license • Perform case management and discharge planning for patients with average daily census of 18-20 • Conduct initial evaluations and develop comprehensive plans of care and care coordination • Utilize InterQual Criteria and Milliman Guidelines for medical necessity determinations • Complete admission criteria assessments and continued stay reviews • Handle appeals and denials, concurrent reviews, and retrospective reviews • Coordinate services including home care, rehab, and long-term care facility referrals • Participate in care conference activities and refer to other disciplines and resources • Document care delivery, plans, goals, and coverage criteria in Epic charting system • Provide biopsychosocial assessments and utilize assessment tools as needed • Work with patients across all age groups including infants, adults, and geriatrics • Handle cases involving dementia, stroke, and other major disease processes • Apply knowledge of regulatory requirements including CMS, HIPAA, ICD-10 coding, and Joint Commission standards • Work on-site and provide remote assistance to affiliate facilities • Float to other sister facilities and affiliate hospitals as needed • Complete required pre-start and annual training modules • Maintain current BCLS certification • Provide minimum of 2 supervisory references covering 1 year of employment • Comply with facility policies, procedures, and dress code requirements • Complete all compliance documentation within required timeframes • Work with patients who have communication problems, community program needs, and high-risk situations • Handle APS (Adult Protective Services) cases and emergency detention situations • Document patient and caregiver teaching and learning activities
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
