Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Master Social Worker to join our growing clinician community. Key Responsibilities:
• Provide social work services as a Licensed Master Social Worker with active WA license
• Conduct initial evaluations and biopsychosocial assessments
• Develop comprehensive plans of care and coordinate patient care services
• Perform discharge planning and care coordination activities
• Utilize InterQual Criteria and Milliman Guidelines for medical necessity determinations
• Complete admission criteria assessments and concurrent reviews
• Conduct continued stay reviews and retrospective reviews
• Handle appeals and denials processes
• Coordinate services with home care and other healthcare providers
• Make referrals to rehabilitation and long-term care facilities
• Participate in care conference activities and interdisciplinary team meetings
• Provide documentation according to plan of care and coverage criteria
• Document care delivery, goals, and patient outcomes
• Utilize Epic and Cerner charting systems for all documentation
• Work with patients across all age groups including infants, adults, and geriatrics
• Manage caseload of 18-20 patients daily census
• Float between sister facilities and affiliate hospitals as needed
• Maintain knowledge of regulatory requirements including CMS, CPT, DRG, HEDIS, HIPAA, ICD-10, NCQA, OSHA, and Joint Commission standards
• Provide services for patients with communication problems and high-risk intervention needs
• Apply knowledge of community programs, APS, CPS, and emergency detention procedures
• Work with major disease processes including dementia, stroke, cancer, CHF, COPD, and psychiatric disorders
• Maintain current BCLS certification
• Complete required pre-start and annual training modules
• Provide minimum of 2 supervisory references covering 1 year of employment experience
• Comply with all facility policies and regulatory standards
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
