Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Apple Valley, CA. Key Responsibilities:
• Assess patients and coordinate care delivery
• Conduct discharge planning and utilization review
• Organize and participate in family meetings
• Participate in the development of treatment plans
• Work with HMOs, IPAs, and other insurance providers
• Provide knowledge and support for adoption/foster care, drug/alcohol abuse treatment, homelessness issues, mental health crises, advance directives, POLST, and protective services processes
• Arrange DME, SNF placement, and ambulance transport independently
• Utilize Epic charting system for computerized documentation
• Manage caseload of up to 25 patients (45 on weekends)
• Work with interdisciplinary support including Physical Therapy and Social Services
• Provide care for patients across all age groups including neonates, adolescents, adults, and geriatrics
• Work minimum of 4 weekend shifts in a 6-week period
• Commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float between all facilities within the state as required
• Complete required modules pre-start and annually (average 16 hours)
• Self-report time spent on non-billable training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Use facility-provided communication devices when required for assignment duties
• Complete skills checklist and maintain required certifications
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
