Position Summary:
VitalHire is seeking a skilled and compassionate SLP - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in New Bedford, MA. Key Responsibilities:
• Provide speech-language pathology services in skilled nursing facility setting
• Conduct comprehensive evaluations and assessments of patients with communication and swallowing disorders
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide direct therapy services for speech, language, cognitive, and swallowing disorders
• Document patient progress and maintain accurate clinical records
• Collaborate with interdisciplinary team members including physicians, nurses, occupational therapists, and physical therapists
• Participate in care plan meetings and team conferences
• Educate patients, families, and caregivers on treatment strategies and home programs
• Monitor patient progress and adjust treatment plans as needed
• Ensure compliance with state regulations and facility policies
• Maintain current state licensure and required continuing education credits
• Work Monday through Friday during day shift hours
• Complete required documentation within established timeframes
Job Requirements:
- Active, unencumbered license in MA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
