Position Summary:
VitalHire is seeking a skilled and compassionate Social Services - MSW (Masters of Social Work) to join our growing clinician community. Key Responsibilities:
• Complete initial and 180-day assessments for participants on assigned panel
• Coordinate hospital discharges by assisting and communicating with hospital discharge planners
• Ensure safe and appropriate discharge plans while communicating IDT recommendations
• Coordinate SNF discharges for participants requiring rehab or skilled needs
• Work within an interdisciplinary team (IDT) for continuity of care
• Provide case management services for geriatric participants
• Conduct biopsychosocial assessments and develop problem lists
• Facilitate discharge and contingency planning
• Provide crisis intervention and individual counseling
• Document care delivery, plans, and goals according to plan of care
• Participate in care conference activities
• Refer participants to other disciplines and resources as needed
• Provide and reinforce individualized participant and caregiver education
• Complete required modules and training programs pre-start and annually
• Utilize Epic charting system for documentation
• Float between facilities within the state as required
• Maintain current BLS certification
• Maintain required social work licensure (WA LICSW, WA LSWAIC, or WA LASW)
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Self-report time spent on non-billable training modules
• Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
