Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading SNF in Lebanon, NH. Key Responsibilities:
• Provide certified nursing assistant services in skilled nursing facility setting
• Work scheduled shifts including Thursday, Friday, Saturday in week 1 and Sunday, Monday, Thursday in week 2
• Participate in every other weekend rotation as assigned
• Provide direct patient care and assistance with activities of daily living
• Maintain current CNA certification and state licensure
• Follow facility policies and procedures for patient care delivery
• Document patient care activities and observations accurately
• Assist patients with mobility, transfers, and positioning
• Support patients with personal hygiene and grooming needs
• Monitor and report changes in patient condition to nursing staff
• Maintain professional standards and ethical conduct
• Participate in required training and continuing education programs
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
