Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading SNF in Rio Rancho, NM. Key Responsibilities:
• Provide skilled nursing care in both skilled nursing facility (SNF) and long-term care (LTC) settings
• Utilize PCC (Point Click Care) system for documentation and patient management
• Complete Medication Administration Records (MAR) and Treatment Administration Records (TAR)
• Work scheduled shifts including Monday and Tuesday in skilled unit and Thursday in long-term care
• Collaborate effectively with medication technicians and other nursing staff
• Maintain excellent communication skills with patients, families, and healthcare team
• Demonstrate comprehensive Plan of Care (POC) knowledge and implementation
• Maintain reliable attendance with no history of no-call/no-show incidents
• Ensure consistent work schedule adherence without frequent call-outs
• Provide direct patient care during day shift hours
• Coordinate care between skilled and long-term care units as assigned
• Maintain current LPN licensure and required certifications
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
