Position Summary:

Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in skilled nursing facility setting • Deliver patient care according to established nursing standards and protocols • Administer medications and treatments as prescribed by physicians • Monitor patient vital signs and document changes in patient condition • Assist with patient assessments and care planning • Collaborate with interdisciplinary healthcare team members • Maintain accurate and timely documentation of patient care activities • Follow infection control procedures and safety protocols • Communicate effectively with patients, families, and healthcare staff • Participate in quality improvement initiatives • Adhere to facility policies and procedures • Maintain current nursing licensure and required certifications
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
