Position Summary:

Key Responsibilities:
• Provide patient care services as a Licensed Practical Nurse (LPN)
• Work skilled nursing shifts on Monday and Tuesday
• Work long-term care (LTC) shifts on Thursday
• Utilize PCC (Patient Care Coordinator) system for patient management
• Complete MAR (Medication Administration Record) documentation
• Complete TAR (Treatment Administration Record) documentation
• Work day shifts alongside medication technician and another nurse
• Demonstrate effective communication skills with patients, families, and healthcare team
• Apply POC (Plan of Care) knowledge in patient care delivery
• Maintain reliable attendance with no history of no-call/no-show incidents
• Maintain consistent work schedule with minimal call-outs
• Maintain current LPN licensure and credentials
• Collaborate effectively with interdisciplinary healthcare team members
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
