Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. Key Responsibilities:
• Provide physical therapy services working 4 ten-hour days per week
• Provide occasional Saturday coverage as needed
• Utilize Epic electronic health record system for documentation
• Complete required license/certificate verification and maintain valid state licensure
• Complete mandatory health screenings including fit test and 9-panel drug screen
• Complete specialty-specific competency testing and annual competency requirements
• Maintain required vaccinations including COVID-19, MMR, Hepatitis B, Varicella, and Tdap
• Complete tuberculosis screening via 2-step PPD or IGRA testing
• Pass criminal background check and complete conflict of interest disclosure
• Complete education verification and professional reference checks
• Adhere to code of conduct and maintain patient confidentiality agreements
• Complete Joint Commission testing and skills checklist assessments
• Participate in required training modules and compliance documentation
• Work scheduled shifts from 08:00 AM to 06:30 PM
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
