Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Evenings at a leading LTC in Moses Lake, WA. Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse on evening shift (2p-10:30pm) • Work 4 on 2 off rotation schedule • Deliver patient care in a secured Alzheimer's/Dementia facility setting • Manage nursing responsibilities for approximately 25 patients per unit • Maintain current LPN licensure and certifications • Provide specialized care for patients with Alzheimer's and Dementia conditions • Follow facility protocols and safety procedures for secured unit operations • Document patient care activities and observations according to facility standards • Collaborate with interdisciplinary team members to ensure comprehensive patient care • Administer medications and treatments as prescribed and within LPN scope of practice
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
