Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Moses Lake, WA. Key Responsibilities:
• Provide nursing care in an Alzheimer's/Dementia Care center • Work on secured units with an average census of 25 patients per unit • Work day shift schedule (6a-2:30p) with 4 on 2 off rotation • Deliver specialized care for patients with Alzheimer's and dementia conditions • Maintain patient safety and security protocols in secured units • Monitor and assess patient conditions throughout shift • Administer medications and treatments as prescribed • Document patient care and observations accurately • Collaborate with interdisciplinary healthcare team • Ensure compliance with facility policies and procedures
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
