Experience
- Minimum 2 years acute medical Care Manager experience performing Care Coordination required
- Previous travel experience required
License & Certification Required
- California state RN license required; pending license accepted if licensed upon start date

- Experience with Care Coordination
- Discharge Planning skills
- Familiarity with CMS: Centers for Medicare and Medicaid Services
- Knowledge of HIPAA guidelines (Health Insurance Portability and Accountability Act)
- Competent with computerized documentation
- Experience with DME (Durable Medical Equipment) arrangements
- Ability to assess patients
- Experience with family meeting organization and participation
- Knowledge of protective services processes
- Experience with patients experiencing homelessness
- Experience with patients with mental health history or in crisis
- Understanding of drug/alcohol abuse treatment options

- Experience with Trauma Level I facilities (preferred)
