Experience
- Minimum 2 years Acute Care Case Management/Discharge Planning experience required
- Previous travel experience required
License & Certification Required
- California state RN license required; pending licenses accepted if licensed upon start date

- Experience with Care Coordination
- Experience with Discharge Planning
- Knowledge of admission criteria and benefits eligibility
- Competence in using computerized documentation systems, specifically Epic
- Experience with utilization review
- Ability to organize and participate in family meetings
- Experience with HMOs, IPAs, and other health services
- Competent in arranging DME, SNF, and ambulance transport independently
Desired Skills
- Knowledge of adoption/foster care and drug/alcohol abuse treatment options
- Familiarity with processes for patients experiencing homelessness or with mental health histories
- Experience with advance directive and POLST
