Experience
- Minimum 2 years acute care case management/discharge planning experience required
- Previous travel experience required
- Must have hospital experience; behavioral health experience does not apply
License & Certification Required
- California state RN license required (pending license accepted if licensed upon start date)
Skills Required
- Experience with assessing patients
- Care coordination experience
- Discharge planning experience
- Utilization review experience
- Competent with arranging DME (Durable Medical Equipment)
- Competent with arranging SNF (Skilled Nursing Facility) and ambulance transport
- Familiarity with HMOs, IPAs, etc.
- Knowledge of adoption/foster care processes
- Knowledge of drug/alcohol abuse treatment options
- Ability to organize and participate in family meetings
- Experience with patients experiencing homelessness
- Experience with patients with mental health history or in crisis
- Knowledge of advance directive and POLST
- Understanding of protective services processes
- Epic charting system experience required
Desired Skills
- Knowledge of CMS (Centers for Medicare and Medicaid Services) processes
- Understanding of Department of Health regulations
- Experience with surgical services
- Familiarity with The Joint Commission/ Core Measure/National Safety Goals
- Experience in benefits eligibility assessment
- Experience with DRG (Diagnosis Related Groups)
- Experience in emergency department or ICU settings
