Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Master Social Worker to join our growing clinician community. Key Responsibilities:
• Provide psychosocial and emotional support to patients and families as a consult-based service
• Complete comprehensive psychosocial assessments using person-centered approach
• Develop appropriate discharge plans in alignment with policy and regulations
• Conduct biopsychosocial assessments and initial evaluations
• Develop plan of care to meet patient and caregiver needs
• Provide brief clinical interventions as appropriate
• Assist with resource navigation and coordination of care
• Screen for suicidality and engage in safety planning when appropriate
• Complete appropriate reports of harm based on state statutes
• Coordinate with outside entities and community resources
• Work collaboratively with RN Case Managers and interdisciplinary care team
• Document pertinent information to facilitate care coordination and delivery
• Document patient response to treatment and interventions
• Facilitate discharge and contingency planning
• Provide crisis intervention services
• Educate patients and caregivers to facilitate safe discharge
• Assist with advance directives and end-of-life planning
• Navigate insurance and benefit programs including Medicaid, Medicare, and social services
• Float between hospital units and affiliate hospitals as needed
• Work with diverse patient populations including pediatrics, adults, and geriatrics
• Support Emergency Department, Children's Hospital, and Adult inpatient teams
• Utilize Epic charting system for documentation
• Complete required training modules pre-start and annually
• Work 2 out of 3 major holidays (Thanksgiving, Christmas, and New Year)
• Maintain current Alaska state licensure as LMSW or LCSW
• Provide minimum of 2 supervisory references covering 1 year of employment
• Adhere to NASW Code of Ethics and professional standards
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
