Position Summary:

Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse (LPN)
• Work assigned shifts including both day and evening schedules
• Follow rotating weekly schedule patterns as assigned
• Deliver patient care according to established nursing protocols and standards
• Document patient information and nursing interventions accurately
• Collaborate with healthcare team members to ensure continuity of care
• Administer medications and treatments as prescribed within LPN scope of practice
• Monitor patient vital signs and report changes to appropriate personnel
• Assist with patient assessments and care planning
• Maintain professional nursing standards and ethical practice
• Complete required nursing documentation in a timely manner
• Participate in shift reporting and communication with incoming staff
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
