Position Summary:
VitalHire is seeking a skilled and compassionate PT - PT to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in West Reading, PA. Key Responsibilities:
• Provide comprehensive physical therapy services to patients across various healthcare settings
• Conduct thorough patient evaluations and assessments to determine appropriate treatment plans
• Develop and implement individualized treatment programs based on patient needs and medical conditions
• Document patient progress and treatment outcomes in accordance with regulatory requirements
• Collaborate with interdisciplinary healthcare teams including physicians, nurses, and other therapists
• Educate patients and families on treatment procedures, home exercise programs, and injury prevention
• Maintain current state licensure and professional certifications as required
• Adhere to facility policies, procedures, and safety protocols
• Participate in continuing education and professional development activities
• Ensure compliance with HIPAA and other patient confidentiality regulations
• Maintain accurate and timely documentation of all patient interactions and treatments
• Work effectively within assigned shift schedule of 8:00 AM to 4:30 PM
Job Requirements:
- Active, unencumbered license in PA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
