Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Assess patients and coordinate care for caseloads up to 25 patients (weekends up to 45) • Perform discharge planning and utilization review for acute care patients • Organize and participate in family meetings as part of treatment planning • Develop and participate in treatment plan creation • Coordinate with HMOs, IPAs, and other insurance entities • Arrange durable medical equipment (DME), skilled nursing facility placements, and ambulance transport independently • Provide case management services for patients with complex needs including adoption/foster care, substance abuse, homelessness, and mental health issues • Handle advance directives, POLST documentation, and protective services processes • Collaborate with interdisciplinary team including Physical Therapy and Social Services • Provide care for patients across all age groups from neonates to geriatrics • Utilize Epic charting system for computerized documentation • Work weekend rotation schedule to meet department needs • Float between facilities within the state as required • Complete required pre-start and annual training modules • Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Maintain current state nursing license • Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty • Use facility-provided communication devices when required for assignment duties • Self-report time spent on non-billable training modules and ensure completion is recorded • Maintain professional appearance in business attire or scrubs as required
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
