Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Nights at a leading SNF in Gallup, NM. Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in skilled nursing facility setting
• Work night shift from 7:00 PM to 7:00 AM
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and document changes in condition
• Assist with activities of daily living and patient mobility
• Maintain accurate patient records and documentation
• Collaborate with interdisciplinary healthcare team members
• Follow facility policies and procedures for patient care delivery
• Ensure patient safety and infection control protocols are maintained
• Communicate effectively with patients, families, and healthcare staff
• Participate in care planning and patient assessments
• Respond to patient emergencies and provide appropriate interventions
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
