Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Seaside, OR. Key Responsibilities:
• Provide physical therapy services in home health setting
• Conduct patient visits with 1:1 ratio, visiting up to 6 patients per day
• Maintain 30 productivity points standard
• Complete OASIS documentation as required
• Utilize EPIC Remote Client for Home Health charting on laptop and Rover phone app
• Provide care for patients with various diagnoses including acute care, rehabilitation, orthopedic, and neurological conditions
• Develop and implement home exercise programs for patients
• Provide patient and family education
• Maintain current BLS certification and state licensure
• Complete required pre-start and annual training modules (average 16 hours, non-billable)
• Self-report time spent on training modules with completion recorded in Workramp transcripts
• Maintain reliable transportation for home visits with occasional travel radius of up to 60 miles per day
• Adhere to business casual dress code (scrubs allowed but not required)
• Float between facilities within the state as required
• Utilize facility-provided communication devices when required for patient care
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty setting
• Comply with COVID-19 vaccination requirements including primary series plus one booster
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
