Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Nights at a leading SNF in Quakertown, PA. Key Responsibilities:
• Work five (8) hour shifts per week on a rotating 2 week schedule with every other weekend
• Provide nursing care during night shift hours (23:00:00 - 07:30:00)
• Deliver patient care services in a skilled nursing facility setting
• Maintain current LPN licensure and certifications
• Follow facility policies and procedures for patient care delivery
• Document patient care activities and maintain accurate medical records
• Administer medications and treatments as prescribed
• Monitor patient vital signs and report changes to appropriate personnel
• Assist with patient mobility and activities of daily living
• Collaborate with healthcare team members to ensure quality patient outcomes
Job Requirements:
- Active, unencumbered license in PA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
