Position Summary:

Key Responsibilities:
• Provide direct patient care and assistance with activities of daily living
• Assist patients with mobility, transfers, and positioning
• Monitor and document vital signs and patient observations
• Help with feeding, bathing, and personal hygiene care
• Maintain accurate documentation of patient care activities
• Follow infection control protocols and safety procedures
• Communicate effectively with nursing staff and healthcare team members
• Respond to patient calls and requests promptly
• Assist with medical equipment and supplies as directed
• Maintain patient dignity and confidentiality at all times
• Complete assigned tasks within designated shift hours
• Adhere to facility policies and procedures
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
