Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading SNF in Lebanon, NH. Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in a Skilled Nursing Facility
• Work alternating weekly schedule: Week 1 (Monday, Tuesday, Saturday) and Week 2 (Sunday, Monday, Tuesday)
• Maintain shift schedule from 06:45 to 19:15
• Administer medications and treatments as prescribed
• Monitor and document patient vital signs and health status
• Assist with activities of daily living for residents
• Collaborate with healthcare team members including RNs, physicians, and other clinical staff
• Maintain accurate and timely documentation of patient care
• Follow facility policies and procedures for infection control and safety
• Provide compassionate care to elderly and rehabilitation patients
• Respond to emergency situations and implement appropriate interventions
• Maintain current LPN license and required certifications
• Participate in care plan meetings and patient assessments
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
