Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Seaside, OR. Key Responsibilities:
• Provide physical therapy services to home health patients in 1:1 ratio
• Visit up to 6 patients per day maintaining 30 productivity points standard
• Utilize Epic Remote Client for Home Health charting and documentation on laptop
• Use Rover phone application for patient care coordination
• Complete OASIS assessments as required
• Maintain current BCLS certification
• Maintain valid state physical therapy licensure
• Provide reliable transportation for patient visits within 60-mile radius
• Complete required modules pre-start and annually (average 16 hours, non-billable)
• Self-report time spent on training modules with completion recorded in Workramp transcripts
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Maintain primary series COVID vaccination plus one booster
• Develop and implement home exercise programs for patients
• Provide patient and family education on treatment plans and care
• Treat various conditions including post-operative cases, spinal conditions, joint replacements, neurological disorders, and traumatic injuries
• Float between facilities within the state as required
• Utilize facility-provided communication devices when required for patient care coordination
• Adhere to business casual dress code requirements
• Work 8-hour day shifts with potential for one call-off per pay period
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
