Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Fair Lawn, NJ. Key Responsibilities:
• Provide direct patient care and assistance with activities of daily living
• Monitor and document patient vital signs and condition changes
• Assist patients with mobility, transfers, and positioning
• Support patients with personal hygiene and grooming needs
• Maintain accurate and timely documentation in patient records
• Communicate effectively with nursing staff and healthcare team members
• Ensure patient safety and implement fall prevention measures
• Assist with feeding and hydration support as needed
• Maintain clean and organized patient care environments
• Follow infection control protocols and safety procedures
• Respond appropriately to patient calls and requests
• Participate in care plan meetings and patient assessments
• Maintain professional boundaries and patient confidentiality
• Complete required training and continuing education requirements
Job Requirements:
- Active, unencumbered license in NJ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
