Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Milford, NH. Key Responsibilities:
• Provide direct patient care as a Certified Nursing Assistant in a skilled nursing facility setting
• Assist patients with activities of daily living including bathing, dressing, and mobility
• Monitor and document patient vital signs and health status
• Support patients with feeding and nutrition needs
• Maintain patient dignity and comfort during care delivery
• Follow established care plans and nursing protocols
• Communicate effectively with nursing staff and healthcare team members
• Ensure patient safety and infection control protocols are maintained
• Document patient care activities accurately and timely
• Assist with patient transfers and positioning
• Respond to patient call lights and requests promptly
• Maintain clean and organized patient care areas
• Work scheduled 8-hour day shifts from 7:00 AM to 3:00 PM
• Adhere to facility policies, procedures, and regulatory requirements
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
