Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Social Worker - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Silver Spring, MD. Key Responsibilities:
• Work as discharge planner for medical/surgical telemetry, ICU, ED, PCU, joint/spine, and observation short stay units
• Provide social work services Monday-Friday 8:00 AM to 4:30 PM with occasional weekends
• Focus exclusively on discharge planning rather than behavioral health services
• Maintain valid state licensure and provide license verification
• Complete required health screenings including fit test, MMR, Hepatitis B, PPD/IGRA testing, and Varicella
• Maintain current COVID-19 and influenza vaccinations
• Complete comprehensive background checks and criminal background verification
• Pass required drug screening and physical/health statement requirements
• Complete mandatory training modules and Joint Commission testing
• Adhere to organizational code of conduct and integrity policies
• Maintain confidentiality of patient information and system security agreements
• Complete skills checklist and competency assessments
• Comply with conflict of interest disclosure requirements
• Participate in education verification and credentialing processes
Job Requirements:
- Active, unencumbered license in MD
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
