Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Assess patients and coordinate care throughout their hospital stay
• Develop and participate in treatment planning with interdisciplinary team
• Conduct discharge planning and arrange appropriate post-acute care services
• Organize and participate in family meetings as needed
• Arrange durable medical equipment (DME), skilled nursing facility placement, and ambulance transport independently
• Work with HMOs, IPAs, and other insurance providers for care authorization
• Provide case management services for patients with complex social situations including homelessness, mental health crises, and substance abuse
• Navigate adoption/foster care, protective services processes, and advance directive discussions
• Complete computerized documentation using Epic charting system
• Maintain knowledge of CMS guidelines, DRG classifications, and HIPAA compliance
• Work with adult and geriatric patient populations with daily census up to 25 patients
• Float between sister facilities and affiliate hospitals as required
• Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Complete required pre-start and annual training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Maintain current state licensure and required certifications
• Utilize facility-provided communication devices when required for assignment duties
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
