Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Monticello, IA. Key Responsibilities:
• Provide physical therapy services as a Physical Therapist
• Work 40 hours per week with 8-4p or 9-5p scheduling
• Work weekends as needed
• Complete license/certificate verification and maintain current licensure
• Complete required drug screening and fit testing
• Adhere to code of conduct and confidentiality agreements
• Complete specialty specific competency testing
• Maintain required vaccinations including COVID-19, MMR, Hepatitis B, PPD/IGRA, Varicella, and Tdap
• Complete physical/health statement requirements
• Complete criminal background check and various verification processes
• Complete Joint Commission testing and skills checklist
• Utilize Epic charting system (preferred but not required)
• Complete child and adult dependent abuse training
• Comply with all facility policies and regulatory requirements
Job Requirements:
- Active, unencumbered license in IA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
