Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Evenings at a leading SNF in Newport, VT. Key Responsibilities:
• Provide direct patient care in a skilled nursing facility environment
• Assist patients with activities of daily living including bathing, dressing, and mobility
• Monitor and record vital signs and patient observations
• Assist with feeding and maintain proper nutrition and hydration for patients
• Help patients with ambulation and transfers using proper body mechanics
• Maintain accurate documentation of patient care activities
• Follow infection control protocols and maintain a clean, safe environment
• Respond to patient call lights and requests promptly
• Assist nursing staff with medical procedures as directed
• Communicate patient status changes to supervising nurses
• Maintain patient dignity and privacy at all times
• Work evening shift from 3:00 PM to 11:00 PM
• Adhere to facility policies and procedures
• Participate in continuing education and training programs
Job Requirements:
- Active, unencumbered license in VT
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
