Position Summary:

Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in skilled nursing facility setting • Work night shift hours from 19:00 to 07:30 • Deliver comprehensive patient care in accordance with nursing standards and facility protocols • Administer medications and treatments as prescribed by physicians • Monitor patient vital signs and document changes in patient condition • Assist with activities of daily living and patient mobility • Maintain accurate and timely documentation of patient care • Collaborate with interdisciplinary healthcare team members • Respond to patient emergencies and provide appropriate interventions • Ensure compliance with state regulations and facility policies • Participate in patient care planning and implementation • Maintain current LPN licensure and required certifications
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
