Position Summary:
VitalHire is seeking a skilled and compassionate SW - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Camden, ME. Key Responsibilities:
• Provide social work services in a skilled nursing facility setting
• Work Monday through Friday schedule from 8:00 AM to 4:30 PM
• Conduct psychosocial assessments and evaluations of residents
• Develop and implement individualized care plans
• Facilitate discharge planning and coordinate with healthcare team members
• Provide counseling and support services to residents and families
• Maintain accurate documentation and records in compliance with regulations
• Participate in interdisciplinary team meetings and care conferences
• Ensure compliance with federal, state, and facility regulations
• Collaborate with medical staff, nursing, and other healthcare professionals
• Provide crisis intervention services as needed
• Assist with admissions process and orientation of new residents
Job Requirements:
- Active, unencumbered license in ME
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
