Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Nights at a leading SNF in Moses Lake, WA. Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in a secured Alzheimer's/Dementia facility
• Work NOC shift from 10:00 PM to 6:30 AM on a 4 on, 2 off rotation
• Care for patients in secured units with 22-30 patients each
• Administer medications and treatments as prescribed
• Monitor patient vital signs and document changes in condition
• Assist with activities of daily living for dementia and Alzheimer's patients
• Maintain accurate patient records and documentation
• Collaborate with healthcare team members and report patient status
• Ensure patient safety in a secured environment
• Follow facility policies and procedures for skilled nursing care
• Respond to patient emergencies and call situations during night shift
• Maintain current LPN licensure and required certifications
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
