Position Summary:

Key Responsibilities:
• Provide case management and discharge planning services as a Licensed Master Social Worker
• Maintain active Washington state MSW license and post-MSW license (LSWAIC or associate clinical license)
• Conduct initial evaluations and develop comprehensive plans of care
• Perform care coordination activities including referrals to rehabilitation and long-term care facilities
• Utilize InterQual Criteria and Milliman Guidelines for medical necessity determinations
• Complete admission criteria assessments and discharge planning processes
• Perform concurrent reviews, continued stay reviews, and retrospective reviews
• Handle appeals and denials related to patient care coverage
• Coordinate services for home care and document information to facilitate care coordination
• Participate in care conference activities and refer patients to other disciplines and resources
• Document care delivery, plans, goals, and coverage criteria in Epic charting system
• Work with patients experiencing communication problems including hearing and speech issues
• Provide services related to community programs for income and insurance assistance
• Handle Adult Protective Services (APS) cases and high-risk intervention situations
• Conduct biopsychosocial assessments and utilize assessment tools as needed
• Work with patients across all age groups including infants, adults, and geriatrics
• Manage cases involving major disease processes including dementia and stroke
• Maintain knowledge of regulatory requirements including CMS, HIPAA, ICD-10 coding, and Joint Commission standards
• Float between sister facilities and affiliate hospitals as needed
• Complete required compliance documentation and training modules pre-start and annually
• Maintain current BCLS certification
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty setting
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
