Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Evenings at a leading SNF in Warwick, RI. Key Responsibilities:
• Provide skilled nursing care in a skilled nursing facility setting
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report changes to supervising RN or physician
• Maintain accurate patient documentation and medical records
• Assist with patient assessments and care plan development
• Provide wound care and dressing changes as needed
• Support patient activities of daily living and mobility assistance
• Collaborate with interdisciplinary healthcare team members
• Ensure compliance with facility policies and procedures
• Maintain current LPN licensure and required certifications
• Work rotating schedule including every other weekend and every other holiday
• Adapt schedule to meet facility staffing needs
• Communicate effectively with patients, families, and healthcare team
• Follow infection control protocols and safety standards
Job Requirements:
- Active, unencumbered license in RI
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
