Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Medford, OR. Key Responsibilities:
• Provide physical therapy services in home health setting
• Conduct patient visits with up to 6 patients per day maintaining 30 productivity points standard
• Perform comprehensive patient assessments and develop treatment plans
• Complete Oasis documentation requirements
• Utilize Epic charting system for patient documentation
• Maintain current BLS certification and state licensure
• Provide patient and family education on treatment plans and home exercise programs
• Travel to patient homes using own reliable transportation
• Complete required pre-start and annual training modules
• Self-report time spent on non-billable training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Float between facilities within the state as required
• Utilize facility-provided communication devices when required
• Adhere to facility policies and procedures for patient care delivery
• Treat patients with various conditions including post-operative care, neurological disorders, and orthopedic conditions
• Demonstrate competency in acute care, outpatient clinic, rehabilitation, and skilled nursing facility settings
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
