Position Summary:

Key Responsibilities:
• Complete Trinity Child and Adult Dependent Abuse Training during onboarding
• Submit Trinity IA Sing Report as required
• Complete Trinity OFAC screening process
• Successfully pass Trinity Skills Checklist assessment
• Maintain current license/certificate verification
• Complete required fit testing for respiratory protection
• Adhere to Trinity Code of Conduct policies
• Complete I-9 employment verification documentation
• Sign confidentiality agreement for patient and property information and system security
• Complete Integrity and Audit Services documentation
• Submit required non-employee upload forms
• Pass Trinity Specialty Specific Competency Testing for respiratory therapy
• Complete color vision screening assessment
• Maintain current COVID-19 vaccination status
• Provide proof of MMR immunization
• Complete Hepatitis B vaccination series or provide immunity documentation
• Complete tuberculosis screening via 2 Step PPD or IGRA testing
• Provide proof of Varicella immunity or vaccination
• Complete physical examination and health statement
• Pass Trinity 9 Panel Drug Screen
• Complete Trinity Profile submission requirements
• Submit Conflict of Interest Disclosure Form
• Pass criminal background check screening
• Provide education verification documentation
• Complete EPLS verification process
• Pass GSA/OIG exclusion screening
• Complete Trinity Joint Commission Testing requirements
• Maintain NSO professional liability coverage
• Complete system access authorization forms
• Answer COVID-19 specific health questionnaire
Job Requirements:
- Active, unencumbered license in IA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
