Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading $facility_type$ in Lebanon, NH. Key Responsibilities:
• Provide licensed practical nursing care in skilled nursing facility setting
• Work every other weekend rotation schedule
• Cover shifts including Monday, Friday, Saturday on Week 1 and Sunday, Monday, Friday on Week 2
• Work 12.5-hour shifts from 06:45 to 19:15
• Deliver comprehensive patient care services to residents
• Maintain current LPN licensure and certifications
• Document patient care activities and observations
• Administer medications as prescribed and per facility protocols
• Monitor patient vital signs and health status
• Collaborate with healthcare team members on patient care plans
• Ensure compliance with skilled nursing facility policies and procedures
• Provide compassionate care to elderly and rehabilitation patients
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
