Position Summary:
VitalHire is seeking a skilled and compassionate OT - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Willows, CA. Key Responsibilities:
• Provide occupational therapy services to patients in accordance with treatment plans
• Conduct patient evaluations and assessments to determine functional limitations
• Develop and implement individualized treatment programs
• Document patient progress and maintain accurate medical records
• Collaborate with interdisciplinary team members including physicians, nurses, and other therapists
• Educate patients and families on therapeutic techniques and safety measures
• Monitor patient response to treatment and modify plans as needed
• Maintain current occupational therapy licensure and certifications
• Participate in team meetings and case conferences
• Ensure compliance with regulatory standards and facility policies
• Provide training and supervision to support staff as appropriate
• Complete required documentation within established timeframes
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
