Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading SNF in Albuquerque, NM. Key Responsibilities:
• Provide licensed practical nursing services in skilled nursing facility setting
• Work 12.5-hour shifts from 07:00 to 19:30
• Deliver comprehensive patient care according to nursing standards
• Maintain current LPN licensure and certifications
• Document patient care activities and observations accurately
• Administer medications per physician orders and facility protocols
• Monitor patient vital signs and report changes to supervising RN
• Assist with activities of daily living for residents
• Collaborate with interdisciplinary healthcare team members
• Follow facility policies and procedures for patient safety
• Maintain patient confidentiality and HIPAA compliance
• Participate in resident care planning and assessments
• Provide compassionate care to elderly and rehabilitating patients
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
