Position Summary:
VitalHire is seeking a skilled and compassionate SLP - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Belen, NM. Key Responsibilities:
• Provide speech-language pathology services to patients
• Conduct comprehensive evaluations and assessments of speech, language, cognitive-communication, and swallowing disorders
• Develop and implement individualized treatment plans based on assessment findings
• Deliver therapeutic interventions for speech, language, cognitive, and swallowing impairments
• Document patient progress and maintain accurate clinical records
• Collaborate with interdisciplinary team members including physicians, nurses, occupational therapists, and physical therapists
• Educate patients and families on treatment goals, strategies, and home programs
• Participate in team meetings and case conferences
• Ensure compliance with regulatory standards and facility policies
• Maintain current state licensure and professional certifications
• Complete required continuing education and competency requirements
• Provide consultation services to staff and other healthcare professionals as needed
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
