Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 16Hrs Days at a leading SNF in Belen, NM. Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in a skilled nursing facility
• Work 16-hour shifts from 06:00 to 22:00
• Deliver comprehensive patient care in accordance with nursing standards and protocols
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and document changes in condition
• Assist with activities of daily living and patient mobility
• Maintain accurate and timely documentation of patient care
• Collaborate with interdisciplinary healthcare team members
• Ensure compliance with facility policies and procedures
• Maintain current LPN licensure and required certifications
• Participate in continuing education and training programs
• Provide compassionate care to residents in skilled nursing environment
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
