Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Sedona, AZ. Key Responsibilities:
• Provide outpatient occupational therapy services to patients with a daily caseload of 8-11 patients • Maintain current AZ occupational therapy license and BLS certification • Obtain and maintain AZ fingerprint clearance card • Work in a multidisciplinary clinic with OT, PT and SLP services • Collaborate with in-network orthopedics, neuro and primary care providers • Utilize Cerner charting system for patient documentation • Work Monday through Friday, 8:00 AM to 5:00 PM schedule • Complete required modules and training programs during orientation week • Adhere to facility quality and patient safety standards including medication administration (BCMA) • Maintain professional dress code standards (business casual) • Provide two professional references covering required timeframes • Complete compliance documentation and certifications within specified deadlines • Report to facility for required testing within 72 hours after signing • Work minimum of 2 out of 3 designated holidays when required • Participate in orientation program during first week of assignment
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
